1. Make sure that you have at least one Formidable form, and at least one form entry in the form you want to merge into a pdf.

4. Choose new layout in “Layout to use”.
5. Add field mappings to your PDF file in “Manage your custom layout here” section.
6. Copy/paste the shortcode or press “Export” button to download merged PDFs.
